The Public Utilities Commission meeting will be held in Room 400, City Hall, 1 Dr. Carlton B. Goodlett Place, San Francisco, CA.  The Commission meeting room is wheelchair accessible.  The closest accessible BART station is the Civic Center Station at United Nations Plaza and Market Street.  Accessible MUNI lines serving this location are: #47 Van Ness, and #71 Haight/Noriega and the F Line to Market and Van Ness and the Metro Stations at Van Ness and Market and at Civic Center.  For information about MUNI accessible services call (415) 923-6142.  There is accessible curbside parking adjacent to City Hall on Grove Street and Van Ness Avenue and in the vicinity of the Veterans Building at 401 Van Ness Avenue adjacent to Davies Hall and the War Memorial Complex.

THE FOLLOWING MATTERS BEFORE THE PUBLIC UTILITIES COMMISSION ARE RECOMMENDED FOR ACTION AS STATED, BY THE GENERAL MANAGER OF PUBLIC UTILITIES AND CITY ATTORNEY WHERE APPLICABLE.

CONSENT CALENDAR

12. All matters listed hereunder constitute a Consent Calendar, are considered to be routine by the San Francisco Public Utilities Commission, and will be acted upon by a single vote of the Commission. There will be no separate discussion of these items unless a member of the Commission or the public so requests, in which event the matter will be removed from the Consent Calendar and considered as a separate item.

a) Staff Recommendation:
Award Professional Services Agreement No. CS-763, Sunol Valley Water Treatment Plant New Treated Water Reservoir, Environmental Analysis Services, to Sunol Valley Reservoir Partners Joint Venture and authorize the General Manager of Public Utilities to execute the agreement for an amount not to exceed $450,000 and with a duration of 670 consecutive calendar days. The contract will be certified and Notice to Proceed issued when the supplemental appropriation for FY 2005-2006 becomes available.

b) Staff Recommendation:
Award Contract No. CW-385, LaSalle Avenue/Racine Lane/Quint Street/Goettingen Street Sewer Replacement, Rehabilitation and Pavement Renovation, in the amount of $797,977 to Shaw Pipelines.

c) Staff Recommendation:
Award Contract No. CW-403; Oceanside Water Pollution Control Plant and Westside Pump Station Electrical Preventive Maintenance, in the amount of $119,500, to Electrical Reliability Services, Inc.

d) Staff Recommendation:
Accept work performed for Contract No. CS-694, Risk Management Services, and authorize final payment in the amount of $8,306.11 to R. W. Beck.

e) Staff Recommendation:
Adopt resolution recognizing the valuable contributions of the California Association of Sanitation Agencies (CASA) over its 50 years of activity and reaffirm the San Francisco Public Utilities Commission’s participation in CASA and support for its mission.

f) Staff Recommendation:
Ratify the Declaration of Emergency made by the General Manager of Public Utilities, dated July 13, 2005, and approved by the President of the Commission, to replace a 15” diameter sewer which failed and caused a portion of Quintara Street to cave in.

PROFESSIONAL SERVICE CONTRACTS

Amend

13. Discussion and possible action related to amending Agreement No. CS-716, Engineering Services, Calaveras Dam Replacement. (Irons)

Staff Recommendation: Approve Amendment No. 1 to Agreement No. CS-716, to perform professional engineering services for Phases 2 and 3 of a five-phase assistance agreement, consisting of Phase 1 – conceptual engineering/design, Phase 2 – detailed design, Phase 3 – final design, Phase 4 – design services during construction, and Phase 5 – start-up/commissioning, previously awarded to URS Corporation; and authorize the General Manager of Public Utilities to execute this amendment increasing the agreement by $8,000,000, for a total agreement amount of $12,000,000, and with a time extension of 730 consecutive calendar days, for a total agreement duration of 2,190 consecutive calendar days.
