Application to Purchase Lead-Free Kitchen Faucet
 
The requirements for purchasing a discounted lead-free kitchen faucet are:
a.) Your property must be located in the City and County of San Francisco and have a valid mailing address;
b.) You are limited to one faucet per household;
c.) You must be replacing a deck-mounted unit (our deck-mounted unit is pictured on this page);
d.) When you pick up your new faucet, you must submit an Agreement Form signed by the property owner along with $10 payment.
 
Please complete the application form below. Fields with asterisks must be completed. We will notify you by email whether your application has been accepted, rejected, or placed on a waiting list. If your application is approved, you can pick up and pay for the faucet at the Customer Service counter (Ground Floor, 1155 Market Street, San Francisco) during regular business hours (8 AM-5 PM Monday-Friday). We will reserve your faucet for a period of two weeks.
 
NOTE: We anticipate that the demand for faucets may exceed our supply. If that occurs, we will place customers on a waiting list and notify them when we have a new supply of faucets. We are giving daycare centers and pre-schools priority over other applications. If you have questions about the availability of faucets or questions about how to fill out this form, please call Customer Service at (415) 551-4730.
 
Please read over the Agreement Form before you complete the applicaton. You must provide your water bill account number. You can find that number at the top of your water bill shown as A in this image of the bill.
